Features
Deadlines, forms and the OSHA 300 log from one injury report
Reference material with citations. You decide recordability and compensability, and you file.
01
No-login mobile report link
Supervisors or employees submit from a phone. Fields are limited to what the forms need: date, time, place, job role, activity, nature and body-part category, whether care was sought, and days away.
Do not enter medical records, diagnoses or treatment notes.
02
Sites and employee roster
Every report attaches to a location and a job role.
03
Guided first-session onboarding
From sign-up to a first report with live deadlines inside the first session.
04
Versioned state rule table
Per jurisdiction: who receives the first report (the carrier, or the state fund in a monopolistic state such as Ohio's BWC), the state's own form, the deadline, working or calendar days, the source citation and a “last verified” date.
Launch jurisdictions: federal OSHA, California, Texas, Florida, New York, Pennsylvania, Illinois, Ohio, North Carolina, Georgia, Michigan and Ontario. More are added after launch, each verified against the agency's own rule.
05
Knowledge-date clock engine with escalating alerts
The knowledge date starts every applicable clock. Email alerts escalate to a backup contact before and at each deadline.
06
California DWC-1 and Ontario Form 7
The DWC-1 one-working-day tracker records that the form was given to the employee. The Ontario Form 7 three-day clock tracks the filing.
07
OSHA clocks
8-hour (fatality) and 24-hour (in-patient hospitalisation, amputation, eye loss) severe-injury prompts link to OSHA's reporting channel, plus the OSHA 301 seven-calendar-day tracker.
State Plan differences are handled for California (Cal/OSHA), Michigan (MIOSHA) and North Carolina (OSH NC). The employer files; the product does not.
08
Pre-filled OSHA 301 and state forms
Built from the report for you to review and file. Only publicly available forms confirmed reproducible are pre-filled.
09
Recordability prompts and the OSHA 300 / 300A
Prompts are cited reference with a “last verified” date. The 300 log builds from your decided cases.
The 300A carries the February 1 to April 30 posting reminder and the annual electronic-submission reminder. The product does not submit to OSHA's Injury Tracking Application.
10
Five-year record
Audit trail and rule-version history, so a past report shows the rule in force at the time. Export the full record as PDF and CSV. Four roles (Admin, Manager, Analyst, Viewer).
Who it is for
EHS / HR manager across several plants
Backup contact on every clock.
Risk and safety manager
Cited recordability decisions.
Safety coordinator
Prepares drafts for review.

What it never does
It never decides recordability or compensability, never gives legal or coverage advice, never files with carriers, states or OSHA, and carries no medical or PHI fields.
