Intake & Investigation
Injury Intake and Investigation Register
Incident facts in, one organized injury record out. Log each report on an intake register limited to the categories the forms ask for (date, time, place, job role, activity, nature, body-part and event category, care sought yes/no, days away). For each report it shows the date a case would need to be on the OSHA 300 Log and 301 if you decide it is recordable, and the first-report rule row for the site's jurisdiction. Track every witness and whether their statement is in, record contributing-factor categories on an investigation register, and follow each corrective action to closure against an owner and a due date you set. A Summary tab counts open reports, outstanding statements and overdue actions as of your chosen date. There are no fields for medical records or treatment notes, and no incidence rates, penalty amounts or industry figures. Every count is computed from your own entries.
Reference tool. You decide recordability and compensability, and you file.
