Ontario Employer Injury Reporting Workbook (WSIB Form 7 and OHSA Notices)
Enter the facts of an Ontario workplace injury and this workbook records the WSIB Form 7 triggers: health care, lost time, less than regular pay, and modified work. For modified work at regular pay it shows the calendar days worked so far, and you enter whether the more-than-seven-day period has passed and the date the obligation began. It then counts the three business days by which WSIB must receive Form 7, skipping weekends and the Ontario holidays on the holiday list. A separate tracker logs the OHSA notices: immediate notice and a 48-hour written report for a critical injury or fatality, and a log for the four-day written notice, which has no rule row in this edition. The Form 7 and 48-hour rows are Verified on WSIB's and Ontario's own pages; the immediate-notice and worker's-copy rows are flagged Unverified. Every date is computed from what you enter.
